About The Position

The Specialist will provide mentorship and guidance during the first six months of active support for initial connection to services and to maintain the self-directed person’s ongoing services at AIM. The Specialist will work in partnership with core operations for program readiness and continuity of self-direction services. The Specialist will participate in person-centered planning meetings and educate the circle of support on current OPWDD Self-Direction regulations, specific policies, and procedures while acting in tandem with core team members to acquaint the representative and circle of support with the unique AIM Self-Direction model. The Specialist will role model connection, community, and education to enhance the opportunity for the circle of support to strengthen their understanding of AIM agency practices for program preparedness. The Specialist will assess diverse personal outcomes on an ongoing and as needed basis providing quality feedback to AIM personnel that fosters the core principles and values of self-determination.

Requirements

  • Associate’s Degree in a Human Services Related Field with 2 years experience working with individuals with disabilities preferred; OR High School Degree with 4 years experience working with individuals with disabilities preferred.
  • Understanding of Self-Direction and OPWDD Programmatic Knowledge
  • Valid NYS Driver’s License acceptable to Agency Insurance standards. Maintain valid insurance and provide insurance card if using your own vehicle.
  • Ability to exhibit/maintain a professional, kind, courteous and patient manner at all times.
  • Strong verbal and written communication skills
  • Reading and math skills at a level to perform job duties
  • Demonstrates interest and ability to make possible the personal outcomes of people receiving services
  • Adherence to agency policies and procedures

Responsibilities

  • Provide knowledge and expertise in Self-Direction/ OPWDD guidelines
  • Teach and support representatives and circle of support the co-employer partnership with AIM Services
  • Support the Self-Direction department by fostering connections within the greater AIM Services departments
  • Mentorship to engage teams in program readiness and continuity of self-direction services
  • Working with core operations to monitor and maintain all required documentation necessary to the function of the Self-Direction program
  • Support initial budget caseloads within the first 6 months to ensure successful ongoing self-directed services
  • Participating as an active member of the individuals’ program team and Circle of Support
  • Flexibility of schedule to ensure individual care and program implementation
  • Attendance in participation in Education and Training core training
  • Any and all other related duties as requested by supervisor
