The Specialist will provide mentorship and guidance during the first six months of active support for initial connection to services and to maintain the self-directed person’s ongoing services at AIM. The Specialist will work in partnership with core operations for program readiness and continuity of self-direction services. The Specialist will participate in person-centered planning meetings and educate the circle of support on current OPWDD Self-Direction regulations, specific policies, and procedures while acting in tandem with core team members to acquaint the representative and circle of support with the unique AIM Self-Direction model. The Specialist will role model connection, community, and education to enhance the opportunity for the circle of support to strengthen their understanding of AIM agency practices for program preparedness. The Specialist will assess diverse personal outcomes on an ongoing and as needed basis providing quality feedback to AIM personnel that fosters the core principles and values of self-determination.
