About The Position

Self-Direction DSP - Blooming Grove Area (Job #SC-JJ) We are seeking a dedicated and compassionate Community Habilitation Worker to join our team. As a Community Habilitation Worker, you will provide essential support and assistance to individuals with special needs. Your role will involve working directly with clients and their families to ensure their well-being and promote their independence.

Requirements

• Experience or knowledge in special education or caregiving is highly preferred
• Proficiency in data collection methods and techniques
• Valid driver's license and reliable transportation for client transportation
• Knowledge of behavioral health principles and strategies
• Experience in direct support or childcare roles is beneficial
• Ability to work effectively with individuals with special needs

Responsibilities

• Provide direct support and care to individuals with special needs, including assisting with daily activities and personal care
• Implement behavior management strategies to promote positive behaviors and reduce challenging behaviors
• Collect data on client progress and behavior using specified methods and tools
• Collaborate with clients Circle of Support to develop and implement individualized care plans
• Assist clients in developing essential life skills, such as communication, socialization, and self-care
• Provide transportation for clients to appointments, activities, or community outings
• Create a safe and supportive environment for clients, ensuring their physical and emotional well-being
• Maintain accurate documentation of client interactions, progress, and any incidents that occur
