About The Position

Under the supervision and direction of the Self Direction Supervisor, the Self Direction (SD) Coordinator, will provide support and guidance to individuals who self-direct their services through a self-directed budget under the auspice of OPWDD using a person-centered approach.

Requirements

  • High School diploma or equivalent required; bachelor’s preferred
  • Minimum two years of experience providing services to individuals with disabilities; experience with self-directed services preferred.
  • Proficient computer skills
  • Proficient writing skills with grasp of basic grammar and spelling
  • Must be efficient, well organized, and have excellent communication skills
  • Must be able to accommodate flexible schedule, some evenings and weekends required

Responsibilities

  • Participates in Launch Meetings, Life Plan and Circle of Support meetings and assists individuals and their families with billing, training, staffing and other service related concerns as needed.
  • Accesses training resources for individuals, families and staff.
  • Reviews staff’s goal data and time and attendance.
  • Reviews, approves, and submits to OPWDD participants Self-Direction Budgets.
  • Reviews and submits participants’ invoices for reimbursement and payments.
  • Works with families, brokers, and vendors to ensure self-hired staff clear all required background checks.
  • Assists individuals and service providers to complete required service documentation.
  • Reviews service documentation to ensure it is complete, accurate and in compliance with all OPWDD, Medicaid and FREEs quality standards.
  • Works with the Human Resources/Training Department to ensure self-hired and agency staff receive required training and any additional training requested by the individual and his/her family or circle of support.
  • Ensures monthly expenditure reports are accurate and submitted for reimbursement.
  • Assists Brokers in helping individuals and families understand and fulfill their employee and service related responsibilities.
  • Maintains records of contacts with people including but not limited to: individuals, family members, and team members.
  • Serves as a point of contact and source of customer service for individuals and their families.
  • Reviews and submits documentation completed by support brokers on behalf of the individuals they are supporting, including but not limit to, Startup Budgets, Staff Action Plans, Life Plans, Level of Care Documents, Full Budgets, etc.
  • Works with individuals receiving services, brokers, state agencies, and families to ensure the individual is receiving all services requested.
  • Ensures that all required documentation (paper and electronic) is completed/processed appropriately, in a timely manner, and meets quality and compliance thresholds.
  • Performs other duties as assigned.
  • Always communicates in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complies fully with FREE’s confidentiality policies by holding in strictest confidence all information pertaining to members, residents, the people we support and their families, clients, providers, contractors, and employees.
  • Meets all safety training requirements; adheres to all safety policies and reports any and all unsafe conditions; takes measures to prevent accidents and/or eliminate conditions that pose a safety or health risk.
  • Coordinates, supports, attends and participates in all required events/ meetings/ reviews/ committees, including ones outside of standard working hours.
  • Meets all job requirements and stays in compliance.
  • Adheres to FREE’s standard of ethical behavior as outlined in the Employee Handbook and Code of Conduct as well as applicable laws and regulations.
  • Ensures compliance with FREE policies, procedures, and operating standards.
  • Implements a continuous quality improvement process for department services and administrative functions.
  • Assists in the development, implementation and administration of Human Resources policies and programs including compliance with Equal Employment Opportunities and Affirmative Action Programs.
