About The Position

Dynamic Fast Paced Case Management within a National Proto Type System of Care Wraparound service model. Provides assessment, development of the Family Support Plan, and linkage and referral to community-based services and resources to assist in the alleviation of assigned families identified needs and/or service barriers.

Facilitates a working partnership with the families, and effective collaboration with community providers to strengthen the overall sustainability of the Family and their functional connectedness long term with supports and resources.

Requirements

• Masters degree in social services or behavioral health field and one (1) year experience working with children and families or Bachelors degree in social services or behavioral health field with at least two (2) years of experience working with children and families.
• Ability to see and hear in emergency situations.
• Ability to communicate orally and in writing.
• Ability to travel independently to various sites in the community.
• Must be able to complete all portions of NAPPI training that are assigned and perform any NAPPI techniques for which the employee has been trained.
• Must be bilingual Spanish
• Eligibility through a Level II background screening with AHCA and DCF
