About The Position

Bay Area Community Resources (BACR) is seeking a compassionate and dedicated Family Advocate/Case Manager to join our team in supporting the healthy development and positive outcomes of youth and their families in West Contra Costa Elementary Schools. This role is a key part of our Community Schools initiative, and the ideal candidate will play an instrumental role in empowering families, providing case management, and connecting families with critical resources in a school-based setting. As a Family Advocate/Case Manager, you will engage with students and families in a trauma-informed manner, assessing their needs, offering resources, and providing support across a wide spectrum of emotional, mental health, and practical challenges. Your work will help create stronger school-family connections, foster resilience, and contribute to the long-term success of students and their families.

Requirements

  • You have hands-on experience working with families in a diverse, urban school setting and are familiar with the challenges faced by West County youth and families.
  • You have strong interpersonal, communication, and organizational skills, and you’re comfortable working with a wide variety of stakeholders, including parents, school staff, and community partners.
  • You have a proven ability to link families to resources, overcoming obstacles to access and ensuring that families get the support they need.
  • You possess cultural humility and a deep commitment to equity and social justice, understanding the value of diverse perspectives and experiences.
  • You enjoy working in a team-oriented environment and can navigate challenges with flexibility, creativity, and professionalism.
  • Spanish language skills are required.

Responsibilities

  • Provide trauma-informed case management to youth and families, including conducting comprehensive risk and needs assessments, creating and implementing service plans, and providing ongoing referrals and follow-up.
  • Link families to necessary community resources (e.g., health, mental health, social services) and help navigate any barriers to accessing these services. Provide advocacy on behalf of families as needed.
  • Lead and facilitate parent/guardian workshops and education sessions focused on topics such as family-school relationships, healthy parenting, conflict resolution, and child development.
  • Offer immediate support and crisis intervention services to families in urgent need, helping to stabilize situations and coordinate appropriate follow-up.
  • Work closely with school staff, the Community School Coordinator, youth therapists, and families to ensure coordinated service delivery. Attend CARE Team meetings and other school-based student assessment processes.
  • Maintain a comprehensive knowledge of available community resources and actively foster partnerships with local organizations to expand service options for families.
  • Maintain accurate, up-to-date case files, statistical reports, and program documentation in line with program evaluation and reporting requirements.
  • Participate in BACR and school site meetings, project planning, supervision, and ongoing training to stay current with best practices and maintain professional development.
