Requirements
- Troubleshooting problems and providing resolution
- Ability to handle a multi-line phone system
- Highly Organized & Detail Oriented

- Previous Care Coordination a plus!
Responsibilities
- Understands and adheres to established Phoenix Home Care & Hospice policies and procedures.
- Creates and maintains Nursing schedules.
- Schedules Nurses according to care plans and staff availability.
- Discerns client services required as outlined in agreements, urgent requests and care plans.
- Enters staff and client information into database.
- Maintains staff and client database
