The Licensed Vocational Nurse (LVN) is responsible for delivering safe, effective, and individualized nursing care to pediatric patients with complex medical needs in a private duty nursing (PDN) setting. The LVN executes the nursing process including assessment, planning, implementation, and evaluation under the supervision of a licensed Registered Nurse or Clinical Supervisor. This role ensures continuity of care and supports the interdisciplinary team in achieving exceptional outcomes for patients and families. Essential Duties & Responsibilities Provide skilled nursing care in accordance with the patients individualized plan of care and physician orders. Perform ongoing clinical assessments and report changes in patient condition promptly to appropriate supervisory personnel. Support and reinforce the plan of care by collaborating with direct care staff, ensuring accurate implementation of prescribed treatment. Educate patients and families on aspects of care, diagnosis, medication administration, and preventative health strategies. Communicate and collaborate with physicians, therapists, and other healthcare professionals to deliver high-quality, coordinated care. Attend and participate in all required meetings, including team conferences, staff meetings, and continuing education in-services. Document care provided accurately, timely, and in accordance with agency policy, state and federal regulations, and national accreditation standards. Comply with clinical supervision protocols and participate in performance improvement initiatives. Provide care in patient homes and/or center-based settings as directed. Perform additional nursing duties as assigned.
