About The Position

Responsible for managing and maintaining comprehensive transportation schedules to ensure timely and efficient travel for PACE participants based on their healthcare needs. Participates as a member of the Interdisciplinary Team (IDT) and acts as a key liaison between participants, caregivers, internal departments, and external transportation vendors to support uninterrupted, high-quality care.

Requirements

  • High School diploma or GED required.
  • Must be able to demonstrate proficiency of Microsoft computer applications. Experience working with Electronic Medical Record system (EMR) preferred.
  • Minimum of one (1) year of documented experience working with a frail or elderly population required.
  • Skilled in communicating with participants, their families, and other staff members; ability to establish a helping, trusting relationship with participants.
  • Ability to follow through with instructions and directions provided by supervisor, RN, QAPI coordinator, primary care physician, and/or medical director.
  • Ability to utilize appropriate safety measures when providing care.
  • Ability to work effectively and harmoniously as part of an interdisciplinary team, the elderly, providers of services, and peers.
  • Well organized, dependable, flexible, and resourceful.
  • Employees must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
  • Job offers are contingent upon a successful pre-employment drug screen, background check, and physical assessment.

Responsibilities

  • In conjunction with the contracted transportation vendor, develop and maintain participant transportation schedules in alignment with care needs and appointments.
  • Coordinate with interdisciplinary team (IDT) members to ensure smooth scheduling and care delivery.
  • Manage all transportation requests, including center visits, external appointments, and dialysis.
  • Input and track return visits, semi-annual assessments, cancellations, and changes.
  • Serve as the central point of contact for families and care teams regarding transportation logistics.
  • Work with vendors to arrange appropriate transportation, obtain usage data, and resolve scheduling issues.
  • Utilize transportation software to provide real-time updates and estimated times of arrival.
  • Perform reminder calls and document interactions in participants' electronic health records (EHR) according to PACE regulations.
  • Ensure transportation services are promptly removed for disenrolled or deceased participants.
  • Support leadership with transportation reports and data analysis.
  • Educate participants on transportation processes and confirm upcoming appointments.
  • Monitor and resolve scheduling conflicts to prevent double-booking or overlaps.
  • Distribute schedules and other necessary information to the appropriate staff and/or participant.
  • Interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Adhere to and support the center's policies, practices, and procedures. Maintain strict participant confidentiality and adhere to all program policies and procedures.
  • Participate in meetings, trainings, and additional duties as assigned, with flexibility in scheduling.
