About The Position

National Church Residences Home Health division located in Southern Ohio is a Home Health & Hospice agency with a team of health care professionals that is unmatched in the area! We are owned and operated by National Church Residences, the nation’s largest provider of affordable senior housing and health care services. We are seeking a Schedule Coordinator who will share in our vision to advance better living and care for seniors! ESSENTIAL FUNCTIONS Collects data base information during the intake process. Effectively assists the client, physician and/or discharging facility to establish an effective home care/hospice plan prior to the client’s transfer to home setting. Communicates with and educates referral sources regarding services via written and telephone contact. Schedules staff for Medicare, Private Duty, and Passport. Serves as a liaison between referral sources and interdisciplinary teams. Investigates, verifies, negotiates and documents coverage from all payor sources. Advocates on behalf of client to ensure receipt of entitled benefits. Tracks all referrals/admissions on weekly referral report. Updates referral sources on status of clients currently receiving service or recently discharged and the client’s response to services received. Communicates appropriate information to team members regarding new admissions and referrals, including client needs such as a plan of care, supplies, and equipment. Provides appropriate documentation of referral per agency policies. NON-ESSENTIAL FUNCTIONS Participates in agency quality assurance performance improvement programs. Communicates necessary information to other departments. Assists in developing and modifying policies and procedures. Attends and participates in regularly scheduled interdisciplinary team meetings to coordinate care plans, follow up on changes and other communication, problem-solve, receive staff support and in-service education. EXPECTATIONS Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, collaborates and participates in policy creation and appropriate problem solving methods. Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG), the Code of Conduct, as well as any facility handbook, including but not limited to, corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws. With respect to Resident Rights, ensures all care is provided with respect and dignity for the residents/clients, their families and the staff. Reports all complaints made by residents/clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/resident property and/or any other corporate compliance, drug free workplace, safe work practices, all federal, state and local regulations and laws. Completes all annual education requirements timely, including but not limited to Compliance, Privacy, Security and HIPAA training and exhibits behavior as set forth by the Code of Conduct in the performance of their duties. Performs other duties as assigned.

Requirements

  • High school diploma or GED.
  • Must have good communication, comprehension, computer and interpersonal skills.
  • Must have the ability to speak, read, write and understand English.
  • Must have working knowledge of Windows, Microsoft Office Suite, and World Wide Web.

Nice To Haves

  • Prior scheduling experience in the health care field preferred.
