The Housing Navigator is responsible for providing outreach services to unsheltered homeless persons in the community and to those referred by mental health providers, hospitals, and social service agencies. The housing navigator will administer assessments, facilitate placement into emergency housing, and connect to social services and permanent housing. The Satellite Housing Navigator will assist clients in obtaining housing readiness documentation and assist with the housing application process.
