About The Position

Under the direction of the Medical Records/HIM Director, the HIM Processing Associate is responsible to collect, compile, process, store, and maintain medical records of hospital and clinic patients in a manner consistent with medical, administrative, ethical, legal, and regulatory requirements of the health care system. Organize, prioritize, locates, pulls, assembles, scans, indexes, quality checks, tracks both electronic and paper records, and sees to the appropriate transport of records requested by other departments in accordance with established standards. Fills in other HIM Areas when needed. Requirements

  • Knowledge of Medical Terminology
  • Previous office experience preferably in a hospital setting preferred
  • Is cooperative in interactions, treating customers with courtesy, respect, and compassion.

  • High School Diploma or Equivalent

Responsibilities

  • Collects all current and previous medical records of discharged patients from all service areas within 24 hours of discharge.
  • Print &Reconcile Daily Discharge List in alpha order
  • Expired patients/Death Certificates
  • Contact Unit for missing discharges-continue process until all have been retrieved-Complete Missing Record Tracking sheet if necessaryssociated emails/process
  • Attach loose documents/ Remove Duplicates/Blanks
  • Review for Advance Directives-bring forward if necessary.
  • Incorporate Consents - check Meditech to ensure Formfast archived
  • Verify Patient information is correct
  • Assemble records in order by service delivery
  • Review record for completeness, authentication and presence of all test results and documentation
  • Validate Discharge Transfer Orders (IP &DSU Records)
  • Enter deficiencies in Meditech from A &A and requested by coding team.
  • Create Temporary Folders for Incomplete process
  • Maintain incomplete records in TDO order by physician
  • Complete secondary analysis
  • Check PDOCs/Dictation Reviews and updates deficiencies as incomplete charts are processed and complete by providers
  • Scanning- All records will be scanned into Meditech
  • Indexing/Quality Assurance-All records will be reviewed and indexed to ensure crossover
  • Telephone/Faxing/Walk-ins/Death Certificates/Healthcare Proxiesll other duties as assigned.
  • Answer telephonessists coworker during periods of heavy workloads
  • Faxes-check for Release of information requests/process on off hours
  • Review Admission Notices for HPC/DNR –Record Level View and 6.0 to 6.1
  • Pull Charts as needed (rarely required)
  • Copy Transfers as needed
  • Re-file records (rarely required)
  • Sane Kits-log and file
  • Order charts off-site storage - Give to Correspondence to obtain from ACCESS
  • Retrieve HCP from Admitting-prep and index if required
  • Death Certificates from Admitting-attach to appropriate record
  • Put away supplies
  • Purge shelves when appropriate/shred paper records
  • Presents a courteous and helpful demeanor, appropriate for all ages, to all patients, visitors and other Steward employees/medical staff members, or any other person an employee encounters while representing Steward Health Care.
  • Handles interruptions in a skillful way.
  • Must be familiar with Steward and Hospital policies and procedures.
  • Complete mandatory education in Steward University as required and timely.
  • Refers any potential medical/legal issues to the Director and Risk Management in a timely manner if necessary.
