About The Position

The Rural Street Outreach and Community Empowerment Case Manager provides flexible, client-centered support across multiple agency locations and rural communities. This position ensures consistent, high-quality service delivery by providing support for case managers as needed, engaging in direct outreach to individuals experiencing homelessness or instability. The role combines case management with field-based outreach, coordination of care, and advocacy to connect clients and patients to appropriate resources and services.

Requirements

  • High School Diploma or equivalent.
  • Valid Ohio Driver's License, clean driving record, reliable transportation, insurable under agency policy.
  • Proficiency in Microsoft Office.
  • Minimum 1 year experience in case management or outreach.
  • Friendly, professional, and courteous demeanor.
  • Strong communication skills; ability to interact productively with senior staff, CAC department staff, community partners, and clients.
  • Ability to obtain QMHS certification within 6 months of employment.
  • Employment contingent upon obtaining and maintaining required credentialing and background checks.

  • Preference given to individuals with a bachelor's or associate degree in social work, counseling, social services, human services, or a related field.
  • Preference given to individuals with experience in mental health, substance use disorder, and/or housing programs.
  • Preference given to individuals with experience serving a non-profit or healthcare entity.

Responsibilities

  • Conduct client intakes, assessments, and service planning for new and existing clients.
  • Assist in gathering, managing, and reporting data related to street outreach activities using the Homeless Information Management System (HIMS) and prepare performance reports through a data reporting tool.
  • Create documents, maintain databases, and send letters, memos, and emails.
  • Assist individuals in person, via phone, virtually, and by email, providing knowledgeable and informed responses.
  • Provide case management services including, at minimum, assessment, referral, and monitoring/follow-up.
  • Maintain a courteous and professional demeanor with customers, staff, funders, and community partners while promoting a positive reflection of the organization.
  • Demonstrate an understanding of systems of care, entitlements and benefits, and cultural awareness.
  • Maintain required records and handle confidential information in a professional manner, including HIPAA compliance.
  • Ensure that all documentation needs for billing are completed within 48 hours of service delivery.
  • Advocate for clients to ensure access to services.
  • Conduct comprehensive intakes for all new referrals, gathering necessary information and assessing needs to ensure appropriate service planning and coordination.
  • Attend meetings, conferences, and training related to the duties of this position, and disseminate information to appropriate individuals.
  • Facilitate referrals to medical, mental health, substance abuse, and social services as appropriate.
  • Collaborate with interdisciplinary teams to coordinate care, ensuring all aspects of the client's needs are addressed.
  • Monitor clients' progress toward achieving their health and social goals, adjusting services as necessary.
  • Maintain accurate, up-to-date case files and documentation in compliance with organizational and regulatory standards.
  • Track and report client outcomes, program performance, and service delivery metrics.
  • Ensure timely and accurate documentation of client interactions, assessments, care plans, and referrals.
  • CAC Committee assignments and other agency service duties as assigned.
  • Attend meetings and conferences as assigned.
  • Other tasks and duties as assigned by supervisor.
