The case manager assists clients in obtaining needed services, education and financial assistance in the community. Essential Duties & Responsibilities Priority 1: Equity and Inclusion Reflect and understand how identities could give advantages in the workplace—even if they are not asked for or earned—while others might experience disadvantages. Priority 2: Case Management Safety planning with all survivors. Responsible for case management services for immediate survivor needs that include referrals for education, education assistance, job training and other public assistance. Assist survivors in locating and securing housing. Provide/arrange for education to include interpersonal violence, financial literacy that includes checkbook management, budgeting and money management and other skills for daily living. Advocate for survivors within the criminal justice system as well as the health care system. Provide referrals to DVIS programs to include legal assistance, protective order assistance and advocacy, counseling services, childcare, group counseling and support groups, and children’s counseling. Assist with protective orders in the DVIS Creek County Office Assist survivors with short and long-term solutions for transportation. Responsible for conducting follow-up. Priority 3: Community Resources, Response and Coordination Identify community resources to meet each individual’s needs, by providing referrals to partner organizations. Assist with coordinating a community response to interpersonal violence. Conduct community outreach and build strong community relationships Maintain a comprehensive directory of local services to include housing, financial assistance, educational assistance, job training, job placement services and childcare resources. Priority 4: Statistical and grant reporting Responsible for keeping statistics and providing reports as requested.
