About The Position

Serves as a primary point of contact for patients accessing services through the RuralCare Connect program. This role supports inbound and outbound communications, appointment coordination, and patient navigation to improve timely access to specialty and preventive care. Works closely with the Nurse Practitioner and Community Outreach team to identify patient needs, remove barriers to care, and ensure appropriate follow-up. This position is primarily based in a call center environment and requires strong communication skills, attention to detail, and a patient-centered approach.

Requirements

  • Minimum 2 years’ experience in a call center, customer service, patient access, or healthcare support role.
  • Experience managing high-volume calls and working with diverse populations.
  • High school diploma or equivalent.
  • Strong verbal communication and active listening skills, with a patient-centered and empathetic approach.
  • Ability to multitask and manage multiple systems while maintaining accuracy and efficiency.
  • Strong organizational skills and attention to detail.
  • Ability to identify and escalate clinical concerns appropriately.
  • Basic computer proficiency, including Microsoft Office and EMR systems.
  • Ability to work collaboratively in a team-based environment.
  • Cultural competence and sensitivity to the needs of rural and underserved populations.
  • Problem-solving skills with the ability to navigate barriers to care.

Nice To Haves

  • Experience in a healthcare call center, patient navigation, care coordination, or scheduling environment.
  • Familiarity with medical terminology and healthcare workflows.
  • Experience using electronic medical records (EMR) systems.
  • Associates degree in healthcare administration, business, communications, or a related field preferred.
