About The Position

Position Summary: The RuralCare Connect Nurse Practitioner (APN) is a key clinical leader within the RuralCare Connect program, supporting both centralized access (call center operations) and community-based outreach. This role provides advanced clinical assessment, triage, patient education, and care coordination to improve access to cardiovascular and specialty services for rural populations. The APN will engage patients telephonically and in-person at community sites, addressing barriers to care, facilitating timely referrals, and promoting continuity across the care continuum. This position requires a combination of on-site call center support and travel to community locations and events within designated rural service areas, with needed flexibility to work occasional evenings or weekends to support outreach initiatives.

Requirements

  • Minimum 3 – 5 years of clinical experience as an APN or RN in a relevant setting, e.g., cardiology, pulmonary, primary care, or ambulatory care).
  • Experience in care coordination, case management, or patient navigation.
  • Demonstrated experience with diverse or underserved populations.
  • Graduate of accredited Nurse Practitioner program (Adult, Family, or Acute Care Preferred).
  • Current unrestricted Nurse Practitioner license in the State of New Jersey.
  • National board certification as a Nurse Practitioner (AANP, ANCC).
  • Current DEA and CDS registration.
  • Basic Life Support (BLS) certification.
  • Valid driver’s license with reliable transportation.
  • Strong clinical assessment and critical thinking skills with the ability to triage and prioritize patient needs effectively.
  • Excellent interpersonal and communication skills with the ability to engage patients, families, and community partners.
  • Ability to work independently in community settings and collaboratively within a multidisciplinary team.
  • High level of cultural competence and sensitivity to the needs of rural and underserved populations.
  • Commitment to people-centered care, access improvement, and health equity.

Nice To Haves

  • Prior experience in community health, rural health, or outreach-based care delivery models.
  • Experience supporting telehealth, triage services, or call center-based clinical operations.
  • Familiarity with cardiovascular and/or pulmonary patient populations.
  • Advanced Cardiovascular Life Support (ACLS) preferred.
