The Center for Independence of the Disabled in New York (CIDNY) is seeking a dedicated Housing Case Manager to assist individuals with disabilities in locating affordable housing options in New York City. This role is crucial in helping clients understand their rights regarding reasonable accommodations and home modifications, as well as providing advocacy services to resolve current housing issues. The Housing Case Manager will also be responsible for providing housing subsidies to individuals transitioning from institutional settings to the community. This position involves conducting regular housing workshops and outreach throughout NYC to educate and assist clients in their housing journeys. The ideal candidate will possess a BA or BS degree with at least two years of experience in locating and securing affordable housing in NYC, or an AA degree with a minimum of five years of relevant experience. A strong knowledge of federal, state, and local housing laws, policies, and practices is essential, along with an understanding of how various housing issues impact people with disabilities. The Housing Case Manager will identify housing opportunities for unstably housed individuals and work directly or indirectly with them to avoid institutional placement or facilitate their transition from institutions to the community. Specific duties include maintaining a list of available housing options, assisting clients with application processes, preparing them for housing interviews, and ensuring secure housing outcomes. The Housing Case Manager will also stay current on benefits and services available to consumers, conduct comprehensive assessments of housing needs, and collaborate with clients to create and review service plans. Compliance with confidentiality and HIPAA requirements is mandatory, as is the completion of all required paperwork for consumer files. The role also involves assisting clients in developing self-advocacy skills and participating in community outreach and advocacy activities.
