About The Position

Responsible for the delivery of professional nursing services which will address the physical, psycho-social, spiritual, cultural, developmental, and age-appropriate needs of each individual patient/family. Provides care with a focus on discharge preparation and instruction, and coordinates discharge planning with other disciplines. Requirements

  • Associate Degree Nursing
  • 2 or more years of bedside nursing experience
  • Licensed Registered Nurse (LRN) - Single State - State Board of Nursing with eligibility to work in the state of KS or compact license
  • Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA)

Responsibilities

  • Supports daily interdisciplinary rounds through real‑time documentation, care board updates, and escalation of care barriers.
  • Supports safe, timely discharges by managing orders, identifying needs, and collaborating with interdisciplinary partners.
  • Validates that patients have clear and concise information and understanding of discharge instructions
  • Identifies and prioritizes patient and family learning needs across diagnoses, medications, procedures, and discharge instructions.
  • Facilitates interdisciplinary communication by coordinating updates, clarifying care plans, and supporting collaboration across clinical teams.
  • Promotes quality and safety by monitoring patient needs, escalating concerns, and supporting organizational improvement initiatives.
  • Provides staff education on discharge process
  • Ensures safety of patients, visitors, colleagues and self
  • May perform Staff Nurse duties as needed
  • Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.
