About The Position

The NC ROOTS Hubs Project Manager is responsible for directing and coordinating the large, statewide, complex, day-to-day operations for a key component of the projected $1B (five-year project) federally funded North Carolina Rural Health Transformation Program (NCRHTP). Foundational to the program is the establishment of North Carolina Organizations Orchestrating Transformation for Sustainability (NC ROOTS) Hubs (Initiative 1). These locally governed, community-tailored networks connect medical, behavioral, and social supports, making it easier for rural residents to access comprehensive care in one place. The NC ROOTS Hub Manager will lead the team responsible for designing, implementing, coordinating, and monitoring the full breadth of NC ROOTS Hub activities, including the initial governance and set up of the hubs and their networks, as well as their implementation of programs related to Chronic Disease, Physical Fitness, Food as Medicine (Initiative 2), and Workforce development (Initiative 4), and Sustainable Financing (Initiative 5). The NC ROOTS Hubs Manager will also be responsible for keeping abreast of related statewide programs and their relevance for NC ROOTS hubs/networks (e.g., synergistic efforts in Division of Mental Health, Office of Rural Health, Division of Health Benefits, or NCDHHS’s Data Office). Develop and operationalize plans that align with the strategic direction set by the RHTP Director and the Department. Manage the federally funded RHTP ROOTS Hubs Initiative and lead the completion of all grant requirements related to the RHTP ROOTS Hubs Initiative, under the direction of the Deputy Director, including timely completion of accurate continuation grant applications, drafting grant progress and final reports, working with DHHS Budget and Analysis and DHHS Controllers Office to submit Final Financial Reports, and any other grant reporting required by RHTP or the Department. Ensure technical excellence for initiative 1, by overseeing and/or providing timely, relevant technical assistance, orientation, education, monitoring and training to assist in meeting internal and external partners’ needs and the Department and grant

Requirements

  • Bachelor’s degree from an appropriately accredited institution and five (5) years of experience in the program area of assignment, including two years of supervisory program experience OR An equivalent combination of training and experience

  • Knowledge of Public, Business, and Rural Health Administration: Comprehensive understanding of public and business administration principles, including operational and policy expertise related to rural healthcare delivery, particularly the hub-and-spoke or hub‑model of coordinated care.
  • Skills in Program Planning, Analysis, and Evaluation: Demonstrated skill in analyzing, evaluating, and monitoring programs, including the ability to use data, assess performance, and provide technical guidance to support large-scale initiatives.
  • Ability to Plan, Direct, and Manage Organizational Operations: Ability to plan and direct fiscal, business, and administrative services for a complex organization, ensuring efficient use of resources and compliance with applicable policies and procedures.
  • Leadership and Interpersonal Skills: Ability to select, train, and supervise staff providing technical assistance and oversight, while developing and maintaining effective working relationships with colleagues, state officials, partners, and the general public.
