RN - Transplant Coordinator

About The Position

Provides coordination/case management for the transplant related activities of patient caseload across the continuum of care – pre-transplant, through post-transplant. Provides patient/family education, coordinates patient evaluation, maintains waiting list registration and maintains patient readiness for deceased donor transplants. Arranges transplant admission, monitors clinical pathway progression and provides post discharge follow-up. The coordinator provides professional/peer education, and participates in the development of patient education programs. The coordinator prepares for, and participates in outreach clinics, with physicians. Participates in recipient selection for placement on the UNOS wait list as part of the transplant committee, then communicates the decision to patient, patients' nephrologist, and dialysis program. Provides inpatient and outpatient clinic transplant education. In the post-transplant patient clinic provides ongoing education, monitoring of labs, and arranges outpatient infusion and radiologic procedures, and any other patient needs. Communicates and sets up initial post-transplant visits with primary transplant physician upon return to recipients home. Assists with program development and awareness, research and other duties as delegated. Individualizes patient care using age-related concepts based on the program population which may include all age groups. Applies age-related concepts to including knowledge of growth and development, safety needs, collection and interpretation of data, treatments, communicating and teaching in the overall care typically required. Requirements

  • Bachelor’s Degree in Nursing
  • Washington Registered Nurse License upon hire
  • Remains up to date on UNOS-OPTN and CMS regulations upon hire
  • 5 years of acute care nursing experience

Nice To Haves

  • Certified Clinic Transplant Coordinator (CCTC) Certification
  • 3 years of current experience in cardiology nursing, including transplantation
