Mercy Hospital's 4A Transitional Care and Discharge Unit is a 12-bed unit that will be opening permanently to reduce Emergency Department holding and to promote better throughput. Patients in this unit consist of those who are transitioning from acute care to home or another care setting and patients who no longer require the intensity of acute hospital care but still need specialized medica, nursing or other care before discharge. As an RN on the team, you will deliver exceptional care, throughout the patient care journey. In this role, you will interact directly with patients and their families, offering the opportunity to enhance their experience and provide healthcare education as needed.

Requirements

  • Graduate from an accredited school of professional nursing (ADN or BSN program)
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross
  • One year of current acute care RN experience in a hospital setting

  • Bachelor's of Science in Nursing (BSN)

Responsibilities

  • Provide direct nursing care.
  • Implement and monitor patient care plans.
  • Monitor, record and communicate patients' conditions.
  • Administer prescribed medications and treatments.
  • Note and carry out physician and nursing orders.
  • Assess and coordinate patients' discharge needs.
  • Educate patients and their families.
