About The Position

As our RN Transition Fellow, you will provide professional nursing care to patients of all ages, adhering to individual needs, physician orders, Hospital policy, and nursing standards. You will also assume responsibility for directing and coordinating care team members, focusing on individualized quality patient care consistent with organizational values. Every day, you will collaborate with the healthcare team to implement comprehensive patient care plans and participate in direct patient care activities, including assessment, medication administration, documentation, and patient education across diverse patient populations. To be successful in this vital role, you will demonstrate an eagerness to learn and apply new knowledge, showing initiative in developing increasing skills and confidence. You will exhibit strong foundational nursing knowledge, critical thinking, and excellent communication skills.

Requirements

  • Graduate of Accredited RN school of Nursing
  • Minimum 2 years of RN experience in an acute care setting
  • Registered Nurse: CA, upon hire
  • Basic Life Support - CPR, upon hire
  • Advanced Cardiac Life Support, upon hire
  • Fire Safety, upon hire

  • Bachelors Of Nursing, upon hire or Masters Of Nursing, upon hire
  • 4 years RN experience

Responsibilities

  • Adheres to established Hospital/departmental policies and procedures; hospital and department specific goals and objectives; quality improvement initiatives; legal, safety, and environmental regulations and standards; infection control standards; the California Nurse Practice Act, ANA Code of Ethics, and Patient's Bill of Rights.
  • Applies clinical knowledge and expertise for coordinating patient care by regularly planning and evaluating the care of patients.
  • Establishes and maintains professional and collaborative relationships with other staff members, physicians, patients, patient family members, and the public as appropriate, while assuring confidentiality of all patient information.
  • Demonstrates effective nursing judgement and a working knowledge of the nursing process.
  • Provides guidance to interdisciplinary team members by supervising nursing interventions and assigning related tasks.
  • Provides leadership by establishing priorities and coordinating nursing care.
  • Documents and communicates all observed symptoms, reactions, treatments and changes in the patient condition to the appropriate member of the interdisciplinary health care team.
  • Provides for teaching based on identified need.
  • Coordinates plan of care with other members of the interdisciplinary health care team to achieve desired goals and outcomes.
  • Communicates the plan of care to the patient and advocates for the active involvement of the patient in the evolution of the plan throughout hospitalization.
  • Conforms to legal requirements of nursing practice and Hospital policies and procedures.
  • Demonstrates a professional, cooperative manner when dealing with all members of the interdisciplinary health care team.
  • Demonstrates understanding of organizational structure and utilizes appropriate channels of communication.
  • Administers medications to assigned patients as directed and assesses and documents patient responses; initiates appropriate interventions per Hospital/Unit policy.
  • Demonstrates an understanding of the approaches to and methods of Continuous Performance Improvement.
  • Assigns duties and oversees unlicensed assistive personnel based on legal scope of practice, regulations, and hospital policy.
  • Collaborates with the supervisory staff to identify own educational and professional growth needs.
  • Functions under the supervision of a Registered Nurse Preceptor until successful completion of a RN transition fellow orientation program is achieved.
  • Actively participates in a structured preceptorship during the orientation/residency program that is designed to assist the RN Fellow in developing their nursing technical, professional, and critical thinking skills.
  • Seeks out learning opportunities in conjunction with their assigned preceptor.
  • Attends and successfully completes all core and specialty classes as required.
  • Completes evaluation processes, residency metrics, and competency validations in a timely manner.
  • Demonstrates an understanding of and ability to follow the chain of command.
  • Actively participates in required looping experiences, mentorship program and debriefing sessions in accordance with defined timelines.
