One California Job Summary and Responsibilities As our RN Transition Fellow, you will provide professional nursing care to patients of all ages, adhering to individual needs, physician orders, Hospital policy, and nursing standards. You will also assume responsibility for directing and coordinating care team members, focusing on individualized quality patient care consistent with organizational values. Every day, you will collaborate with the healthcare team to implement comprehensive patient care plans and participate in direct patient care activities, including assessment, medication administration, documentation, and patient education across diverse patient populations. To be successful in this vital role, you will demonstrate an eagerness to learn and apply new knowledge, showing initiative in developing increasing skills and confidence. You will exhibit strong foundational nursing knowledge, critical thinking, and excellent communication skills.
