Job Summary: Provides nursing care in accordance with the substance abuse patient’s plan of care to include comprehensive health and psychosocial evaluation. Responsibilities include monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.

Responsibilities

  • Provide adequate patient assessments including health history.
  • Identify patient care requirements.
  • Communicate effectively with the interdisciplinary team to discuss, assess, and find the most productive way to address patient issues and needs.
  • Takes and records patient vital signs
  • Performs and monitors random drug screening/analysis and other CLIA waived testing
  • Assesses patient symptoms regularly, reporting condition changes to physician if a change in care plan may be necessary
  • Adheres to accessibility, safety, and infection control standards.
  • Supervises administration of medication and monitoring and/or delegates to licensed practical nurses.
  • Monitors on-site narcotics to prevent theft or abuse
  • Administer medication as ordered and evaluate effectiveness through documentation.
  • Implement verbal doctor orders and authenticate these orders in writing within the required timeframe in the patient’s medical records.
  • Schedule and administer TB tests.
  • Schedule and follow up on patient lab work when needed.
  • Order and maintain nursing supplies.
  • Assist patients to help create a path to meeting their individual goals.
  • Ensure informed consent.
  • Maintain patient confidentiality.
  • Provide compassionate support to patients through each phase of their recovery.
  • Actively listen to the concerns and needs of the patient.
  • Maintain timely documentation.
  • Attend treatment team meetings, case conferences, and staff meetings.
  • Participate in personal development activities in order to keep up to date on healthcare practices.
  • Participates in audits/surveys conducted by CARF, DWIHN, DEA, etc.
  • Maintain current professional license with the state.
  • Assist with tasks within the scope of nursing and program services.
  • Assist with any nursing functions associated with clinic research studies.
  • Performs all other tasks as assigned
