Responsibilities Located on the Community Regional campus, Burnett Extended Care Center is our brand-new 154-bed transitional care facility. Purpose-built to serve patients transitioning from hospital to home, Burnett includes specialized units — Memory Care, Long-Term Care, and Transitional Care — that focus on quality, comfort and continuity of care. This environment offers nurses the opportunity to provide patient-centered care across diverse needs in a collaborative, post-acute setting. Provide direct nursing care to patients recovering from illness, surgery, or hospitalization , many with complex medical or rehabilitative needs. Collaborate with an interdisciplinary team to develop and implement individualized care plans that support patient goals and safe transitions of care. Monitor and assess patient conditions, administer medications, and document care in accordance with established policies and standards. Foster a supportive, team-oriented environment that promotes collaboration, open communication, and continuous learning. Requirements

  • Current State of California Registered Nurse license
  • Current Basic Life Support for Healthcare Providers by American Heart Association

Responsibilities

  • Provide direct nursing care to patients recovering from illness, surgery, or hospitalization , many with complex medical or rehabilitative needs.
  • Collaborate with an interdisciplinary team to develop and implement individualized care plans that support patient goals and safe transitions of care.
  • Monitor and assess patient conditions, administer medications, and document care in accordance with established policies and standards.
  • Foster a supportive, team-oriented environment that promotes collaboration, open communication, and continuous learning.
