About The Position

The Quality Audit Specialist (RN) is responsible for reviewing, analyzing, and auditing hospice clinical documentation to ensure accurate and compliant completion of the HOPE (Hospice Outcomes and Patient Evaluation) required documentation. This role supports regulatory readiness and drives quality improvement by monitoring clinical records, evaluating outcome data, and supporting hospice clinicians in meeting CMS standards. The Quality Audit Specialist helps ensure that hospice patients receive high‑quality, person-centered care through accurate assessment and documentation processes.

Requirements

  • Experience with EMR platforms and clinical documentation
  • Strong understanding of CMS hospice requirements and the HOPE assessment tool.
  • Excellent attention to detail, analytical skills, and clinical judgment.
  • Strong communication and coaching abilities.
  • Knowledge of hospice philosophy and end-of-life patient care needs.
  • Ability to review documentation objectively and provide constructive feedback.
  • Strong organizational and time‑management skills.
  • Ability to maintain confidentiality and adhere to HIPAA regulations.
  • Proficiency in data review, reporting, and trend analysis.
  • Active, unrestricted Registered Nurse (RN) license.

Nice To Haves

  • Minimum 2 years of hospice or palliative care nursing experience.
  • Experience in clinical auditing, quality management, or compliance.
  • Prior experience preparing for CMS surveys or accreditation visits.

Responsibilities

  • Performs detailed audits of hospice records to ensure accurate completion of HOPE assessments in accordance with CMS requirements.
  • Validates alignment between HOPE data, narrative documentation, and the patient’s clinical condition.
  • Identifies documentation gaps, scoring inaccuracies, and inconsistencies that may impact quality scores or regulatory compliance.
  • Ensures hospice documentation meets all CMS Hospice Conditions of Participation (CoPs) and agency standards.
  • Monitors timeliness of HOPE submissions and flags missing or delayed assessments.
  • Reviews audit findings and collaborates with hospice leadership to create corrective action plans.
  • Tracks trends in documentation quality, compliance performance, and recurrent issues.
  • Supports agency preparation for CMS surveys, accrediting body reviews, and internal audits.
  • Participates in root-cause analysis and quality improvement initiatives related to clinical accuracy and patient outcomes.
  • Provides education to hospice nurses, social workers, and interdisciplinary team members on HOPE assessment guidelines.
  • Conducts one‑on‑one or group training sessions to enhance documentation quality.
  • Serves as a resource for clinical staff regarding HOPE scoring questions and compliance expectations.
  • Assists with analyzing hospice quality measures tied to HOPE outcomes.
  • Prepares reports for leadership outlining trends, risks, and improvement recommendations.
  • Supports data accuracy for agency quality dashboards and submission requirements.
  • Participates in interdisciplinary quality meetings as needed.
  • Assists with auditing of hospice records for quality measures.
  • Assists with trending deficiencies of documentation for quality measures.
  • Assists with preparing reports and presentations of quality data.
  • Assists with policy development and updates related to HOPE and clinical documentation.
  • Completes special projects assigned by Quality or Clinical leadership.
  • Other duties as assigned.
