RN Patient Navigator - Diagnostic & Procedural Services

About The Position

The nurse in this role will work in clinic to coordinate and streamline patient care, especially patients who are scheduled to have a procedure. They will also be required to travel to the following clinic sites: Chagrin Minoff and Mentor. Participate and collaborate within assigned program(s); staff, leadership and committee at hospital entity and with UH system initiatives. Monitor and guide the patients through the complete experience beginning in the pre-op setting, perform clinical screening of the patient, coordination of evaluation, scheduling of appointments and appropriate testing/imaging, preparation for procedure(s) scheduling, during hospitalization, rehabilitation if applicable and the post-op surgeon/nurse visit and communication. • Facilitate interdisciplinary collaboration in each patient care setting (i.e. hospital, home care, outpatient) to achieve optimal patient outcomes and satisfaction. • Monitor and as needed trouble-shoot the key patient “touch-points” along the continuum. • Document delegated portions of care/interventions in the ambulatory and/or inpatient medical record Participate and collaborate with Patient Education. Develop and monitor the effectiveness of informational and educational material/opportunities. Support and serve as a resource to patients/families. Support Operational Initiatives. Hours: This position is Tuesday-Friday with Monday's off. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.

Requirements

  • (BSN) Bachelor's Degree in Nursing (Required) or (ADN) Associate's Degree in Nursing (Required)
  • 5+ years In clinical medicine (Required) or 3+ years Cardiology experience (Required)
  • Understanding of hospital discharge process; i.e. DME, home care, skilled nursing, etc (Required proficiency)
  • Self-directed and motivated (Required proficiency)
  • Detail oriented (Required proficiency)
  • Strong communication skills (Required proficiency)
  • Familiar with Microsoft Office (Required proficiency)
  • Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire)
  • Basic Life Support (BLS) (Required)

Nice To Haves

  • Ambulatory nursing experience (Preferred)
  • Current knowledge of cardiovascular disease management, trends, care, etc. across the different practice settings. (Preferred proficiency)
  • Advanced Cardiac Life Support (ACLS) May be required based on clinical area standards.
  • Pediatric Advanced Life Support (PALS) May be required based on clinical area standards.
