Hiring Salary Range: RN/BSN: $35.08-$37.72 per hour – Full Salary for position - $35.08-$49.60 per hour RN-AD OR Social Worker: $33.36-$35.86 per hour – Full Salary for position -$33.36-$47.06 This position will be eligible for Clay County’s benefit package, along with opportunities for career development and personal growth. Requirements

Bachelor’s degree in nursing or social work preferred
Licensed in Minnesota as a registered nurse OR social work license preferred
1-3 years of related work experience
Valid driver’s license
Ability to pass a background check
Clay County Application must be submitted for all County positions.

Responsibilities

Provide care coordination to individuals eligible for Special Needs Basic Care (SNBC) through UCare; includes assisting members with establishing in-home services, coordinating medical appointments and transportation, helping members navigate complex health systems, assessing for additional resources to meet the person’s needs.
Provide all required documentation by preparing and maintaining all client records, documenting service and contact activities according to Federal, State Statutes/Rules as well as County policies/guidelines to allow for quality assurance review and maximization of funding.
Other duties as assigned.
