Requirements

Associate's Degree in Nursing Required
6 months of relevant clinical experience Required
Licensed as a Registered Nurse (RN) in the state of WI or holds a license issued by a jurisdiction that has adopted the nurse licensure compact Upon Hire Required
CPR/BLS Upon Hire Required

Responsibilities

Serve as the initial clinical point of contact for patients referred to UW Health | Carbone Cancer Center, including new cancer diagnoses, existing diagnoses, and internal specialty referrals.
Conduct telephone and portal-based assessments, gathering clinical history and understanding each patient’s current plan of care.
Perform high-level clinical triage, identify urgency of referrals, and determine appropriate oncology specialty and care team needs.
Ensure all necessary records, imaging, pathology, and diagnostic testing are obtained prior to consultation.
Coordinate multidisciplinary care, including scheduling consultations, procedures, and diagnostic tests across multiple departments.
Provide education, psychosocial support, and guidance to patients and families during the intake process.
