About The Position

The primary purpose of your job position is to supervise the day-to-day nursing activities of the facility during your tour of duty. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director Of Nursing , to ensure that the highest degree of quality care is maintained at all times. Delegation of Authority As Nurse Supervisor you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. The Supervisor works under the direction of the Director of Nursing and the Assistant Director of Nursing. Responds to all departmental issues requiring managerial intervention on weekends, holidays, off-shifts in the absence of regular department supervisors. Reports significant incidents/issues to Nurse-On-Call for follow-up and/or further intervention with designated administrative personnel. Requirements

Must be a graduate of an approved RN program.
Must possess a current, unencumbered, active license to practice as an RN in this state.
Must be able to read, write, speak, and understand the English language.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident's condition.
Must not pose a direct threat to the health or safety of other individuals in the workplace.
Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must be in good general health and demonstrate emotional stability.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to lift, push, pull, and move a minimum of 50 pounds.
Must be able to perform tasks that may involve exposure to the resident's blood/body fluids.
Must be able to assist in the evacuation of residents.
Must be able to perform CPR and emergency medical treatment.

Nice To Haves

1 year of experience in a hospital, long term care facility, or other related healthcare facility is preferred.

Responsibilities

Assist the Director of Nursing in directing the day-to-day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
Participate in developing, maintaining, and updating written policies and procedures that govern the day-to-day functions of the nursing service department.
Ensure that reference material (i.e.; PDR's, Federal and State Regulations, etc.) maintained at the nurses' stations is current. Recommend written material that will assist the nursing service department in meeting the day-to-day needs of the resident.
Make written and oral reports/recommendations to the Director as necessary/required, concerning the operation of the nursing service department.
Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents' total regimen of care.
Ensure that all nursing service personnel are in compliance with their respective job descriptions.
Ensure compliance with state, federal and JCAHO standards as pertinent to nursing services.
Participate in surveys (inspections) made by authorized agencies as requested by the Executive Director or Director Of Nursing .
Assist in planning the nursing services portion of the resident's discharge plans as necessary.
Meet with the nursing staff, as well as support personnel, in planning the department's services, programs, and activities.
Admit, transfer, and discharge residents as required.
Complete accident/incident reports.
Interpret the department's policies and procedures to personnel, residents, visitors, and government agencies as required.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
Complete written report shift to shift & give verbal report to upcoming staff.
Give verbal report at morning meeting.
Attend pertinent committee meetings of the facility as assigned.
Assist in preparing written and/or oral reports of the nursing service programs and activities to submit to such committee(s) as directed.
Provide information to the Continuous Quality Improvement Committee as requested.
Implement recommendations from establishing committees as instructed by the Director of Nurse Service.
Review daily staffing for sufficient staff & make changes as needed.
Assist in the orientation of newly assigned personnel as directed.
Ensure that a sufficient number of licensed staff are available for your tour of duty to ensure that quality care is maintained.
Ensure that a sufficient number of certified nursing assistants are available for your tour of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident.
Develop work assignments for charge nursed & CNA’s, and/or assist in completing and performing such tasks.
Delegate to each Charge Nurse the administrative authority, responsibility, and accountability necessary to perform his/her assigned duties.
Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse.
Meet with nurses & CNA’s as needed to address concerns.
Make daily rounds of the nursing service department to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to the Director.
Ensure that established policies and procedures, including dress codes, are followed by all departmental personnel.
Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director as required or as may be necessary.
Take part in the process of investigation of & education for complaints & grievances as delegated by DON or ADON.
Develop and maintain a good working rapport with inter-department personnel, as well as other departments within the facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the resident.
Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
Meet with your shift on a regularly scheduled basis to assist in identifying and correcting problem areas and/or to assist in the improvement of services.
Review and evaluate your department's work force and make recommendations to the Director.
Participate in employee performance evaluations, determining your shift's staffing requirements, and making recommendations to the Director.
Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, sex, age, handicap, or marital status.
Take, report, document and assist with covering staff call outs as needed.
Responsible for residents needs on RCH unit during night shift (10pm – 7am).
Inform nursing service personnel of new admissions, their expected time of arrival, room assignment, etc.
Ensure that rooms are ready for new admissions.
Make rounds with physicians as necessary.
Encourage attending physicians to record and sign progress notes, physicians' orders, etc., on a timely basis and in accordance with current regulations.
Inform the Director when physician visits are not made in a timely manner.
Consult with the resident, his/her family, and the resident's physician in planning the resident's care, treatment, rehabilitation, etc., as necessary.
Notify the resident's attending physicians if there is a significant change in the condition of the resident or if the resident required immediate medical care, or notification of the medical director if the resident's attending physician does not respond promptly.
Notify the resident's next-of-kin when there is a change in the resident's condition.
Make rounds on a daily basis in order to observe and evaluate each resident's physical and emotional status.
Review nurses' notes to ensure that they are informative and descriptive of the nursing care being provided, that they reflect the resident's response to the care, and that such care is provided in accordance with the resident's wishes.
Provide direct nursing care as necessary.
Obtain lab tests as ordered.
Report problem areas to the Director. Assist in developing and implementing corrective action.
Keep the Director informed of the status of residents and other related matters through written/oral reports.
Assist the Charge Nurse in monitoring seriously ill residents.
Monitor the use of restraints when necessary and in accordance with our facilities policies and procedures.
Ensure that all RN's and LPN's on your shift comply with written procedures for the administration, storage, and control of medications and supplies.
Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.
Review medication kardex for completeness of information, accuracy in the transcription of physician orders, and adherence to stop order policies.
Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner.
Ensure that residents who are unable to call for help are provided with a mechanism for monitoring needs.
Meet with residents, and/or family members, as necessary. Report problems to the Director.
Assist in arranging transportation for discharged residents as necessary.
Assist in the evacuation of residents during emergency situations.
Perform CPR & medical treatment when needed.
Implement Disaster Plan Policy & Procedure when needed.
Participate in developing, planning, conducting, and scheduling in-service training classes that provide instructions on "how to do the job", and ensure a well-educated nursing service department.
Maintain an effective orientation program as developed by the staff development director that orients the new employee to your shift, its policies and procedures, and to his/her job position and duties.
Provide leadership training that includes the administrative and supervisory principles essential for your Charge Nurses.
Encourage your Charge Nurses to attend and participate in outside training programs.
Maintain IV training & certification.
Assist the training coordinator in providing annual mandated OSHA and CDC in-service training programs for hazard communications, TB management, and blood borne pathogens standards.
Recommend to the training coordinator the in-service training programs you feel will meet the needs of the nursing staff.
Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
Assist in developing safety standards for the nursing service department as necessary.
Assist the Infection Control Nurse in identifying, evaluating, and classifying routine and job-related functions to ensure that tasks in which there is potential exposure to blood/body fluids are properly identified and recorded.
Ensure that an adequate supply of personal protective equipment is on hand and readily available to personnel who perform procedures that involve exposure to blood or body fluids.
Ensure that personnel follow established procedures for the use and disposal of personal protective equipment.
Monitor your shift's personnel to ensure that they are following established safety regulations in the use of equipment and supplies.
Ensure that nursing service work areas (i.e.; nurses' stations, medicine preparation rooms, etc) are maintained in a clean and sanitary manner.
Ensure that all resident care rooms, treatment areas, etc., are maintained in a clean, safe, and sanitary manner.
Ensure that nursing service personnel follow established handwashing procedures.
Ensure that nursing service personnel participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.
Participate in the development, implementation, and maintenance of the infection control program for monitoring communicable and/or infectious diseases among the residents and personnel.
Ensure that nursing service personnel follow established infection control procedures when isolation precautions become necessary.
Participate in the maintenance of the procedures for reporting hazardous conditions or equipment.
Ensure that all personnel wear and/or use safety equipment and supplies (e.g.; back brace, lifter, etc.) when lifting or moving residents.
Ensure that any accidents/incidents/safety violations are reported promptly and in accordance with facility policies and procedures.
Ensure that immediate action is taken to remedy any potential safety hazard.
Ensure that any safety problems, incidents are fully investigated in accordance with facility policies and procedures.
Recommend to the Director the equipment and supply needs of the shift.
Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained on premises at all times to meet the needs of the resident.
Ensure that the procedures for the safe operation of all nursing equipment.
Ensure that only trained and authorized personnel operate the nursing service department's equipment.
Monitor nursing procedures to ensure that nursing service supplies are used in an efficient manner to avoid waste.
Correctly use all necessary equipment for your position (ie, thermoscan, felac, BP cuff, baumanometer, sara/maxi lifs, whirlpool tubs, recliners, wheelchairs,blood glucose testing & control, tube feeding, O2 machines, suction machine, pace maker checks, Pulse Oximeter calibration, etc).
Correctly use facility equipment (ie nurse call system, telephone system, paging system, walkie talkie, computer system, etc.)
Use only the equipment you have been trained to use.
Operate all equipment in a safe manner.
Use only the equipment and supplies necessary to do the job. Do not be wasteful.
Participate in the development of preliminary and comprehensive assessments of the nursing needs of each resident as necessary.
Encourage the resident to participate in the development and review of his/her care plan.
Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
Ensure that all personnel involved in providing care to the residents are aware of the resident's care plan. Ensure that nursing personnel refer to the resident's care plan prior to administering daily care to the resident.
Review nurses' notes to determine if the care is being followed.
Assist the Resident Assessment/Care Plan Coordinator in planning, scheduling, and revising the MDS, including the implementation of RAPs and Triggers.
Review resident's medical and nursing treatments to ensure that they are provided in accordance with the resident's care plan and wishes.
Update care plans as needed.
Keep abreast of economic conditions/situations, and recommend to the Director adjustments in nursing services that ensure the continued ability to provide quality care.
Recommend to the Director the equipment needs of the nursing service department.
Maintain the confidentiality of all resident care information.
Monitor nursing care to ensure that all residents are treated fairly, and with kindness, dignity, and respect.
Ensure that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident's room.
Ensure that all nursing service personnel are knowledgeable of the residents' responsibilities and rights including the right to refuse treatment.
Review complaints and grievances make by the resident and make a written/oral report to the Director indicating what action(s) were taken to resolve the complaint or grievance. Follow facility's established procedures.
Maintain a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.
Report and investigate all allegations of resident abuse and/or misappropriation of resident property.
Ensure that nursing staff personnel honor the resident's refusal of treatment request. Ensure that such requests are in accordance with the facility's policies governing advance directives.
