About The Position

RVNAhealth is seeking a self-motivated, compassionate RN to join our Nurse-Family Partnership (NFP) team. This evidence-based, community health program transforms the lives of parents and their children by providing vital home-based nursing support from pregnancy through the child's second birthday. As a Nurse Home Visitor, you will work directly with eligible individuals and families, delivering nursing education in the home setting. You'll play a pivotal role in promoting healthier pregnancies, stronger parent-child relationships, early childhood development, and long-term family success. Position Highlights: Visit clients in their homes Work remotely between visits Travel required within the Waterbury/Danbury region Flexible schedule Full-Time schedule of 37.5 hour per week Occasional evening or weekend visits required to meet client needs (scheduled in advance by nurse; flex time provided) Work with diverse populations impacted by economic and health disparities Spanish-speaking candidates highly preferred Education-based nursing position; minimal hands-on nursing care Extensive training and support are provided

Requirements

  • Current RN license in good standing and CPR certification required at time of hire.
  • Two years of recent experience in maternal/child health, public health, behavioral health, or home visiting preferred.
  • Excellent written, verbal, and basic computer skills.
  • Individual must be self-directed, enjoy flexibility, and be comfortable with variety and unpredictability in their daily work.
  • Must have a valid driver's license and vehicle to travel throughout the specified area.

Nice To Haves

  • Spanish language fluency highly preferred.

Responsibilities

  • Provide regular home visits to pregnant individuals and their families
  • Educate clients on prenatal health, nutrition, infant care, child development, and goal setting
  • Adhere to the NFP model and the nursing process
  • Carry a caseload of up to 25 clients
  • Participate in initial and ongoing NFP education
  • Collaborate with interdisciplinary partners and community resources
