About The Position
Agawam Rehab & Nursing Campus is seeking an experienced RN Medication Management Nurse for a full-time role in our skilled nursing facility - earn up to $107,000 annually in a non-bedside position with no clinical on-call. This role focuses on medication coordination, therapeutic interchange, and collaboration with pharmacy and physicians to support safe and effective medication management for residents.
Requirements
- Active RN license required
- Experience in skilled nursing, post-acute care, or medication management preferred
- Strong attention to detail and communication skills
Responsibilities
- Ensure medications align with facility formulary and medication profile
- Coordinate therapeutic interchange processes with pharmacy and physicians
- Manage prior authorizations and medication adjustments
- Communicate medication changes with clinical team
- Support compliance with medication-related regulations
- Maintain accurate documentation
