The RN Market Manager plays a highly visible and impactful role, partnering closely with older adults and their families while also serving as a trusted ambassador to the professional, healthcare, and long term care communities. This leader helps ensure referral partners and prospective clients understand not only the services we provide, but the philosophy of care that sets Commonwise apart. What Does Commonwise Do? Commonwise is a full-service in-home care provider. We provide hands-on assistance with homemaking and activities of daily living (ADLs), along with care coordination and nurse-led care management that helps families navigate the medical, emotional, and logistical complexities of aging. Responsibilities

  • Conduct comprehensive in-home client assessments
  • Develop, implement, and update individualized care plans
  • Monitor ongoing care and adjust plans as client's needs change
  • Provide clinical guidance and support during care transitions or urgent situations
  • Coordinate care with clients, families, Caregivers, and healthcare providers
  • Represent Commonwise Home Care within the Williamsburg community
  • Build and maintain relationships with referral partners and community organizations
  • Participate in local events, meetings, and educational activities
  • Collaborate with internal teams to support local growth and referral efforts
  • Assist with caregiver recruitment and onboarding efforts as needed
  • Provide clinical training, mentorship, and ongoing support to Caregivers
  • Conduct performance check-ins to ensure care quality and adherence to standards
  • Collaborate with the Care Coordination team to align clients and Caregivers effectively
  • Maintain regular communication with clients and families to assess satisfaction
  • Address concerns and support positive care experiences
  • Partner with internal teams to support care delivery and operational needs
  • Serve as the local clinical resource for the Williamsburg area
  • Participate in team meetings and contribute to process improvement efforts
