About The Position
1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes Grand Rapids, Battle Creek and Kalamazoo areas. The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients
Responsibilities
- Perform start of care visits and establish care plans. Monthly follow up visits.
- Perform Supervisory Visits to client and train and supervise caregivers
- Collaborate with interdisciplinary groups and coordinate care
- Handle emergency phone lines on off hours
- Participate in community marketing and customer relation activities
- In emergency situations, fill in for field staff and perform CNA and nurse shift duties with additional pay after normal business hours
- On occasion, assist Clinical Manager with Quality Assurance chart audits
