About The Position

Crossroads Health, a 501(c)(3) non-profit organization with facilities located in Mentor, Painesville, and Cleveland, serves Northeast Ohio communities with comprehensive behavioral and primary integrated healthcare, early childhood services, extended housing and recovery services—no matter one’s ability to pay. Our mission is to provide hope, healing, and healthcare to everyone. The Integrated Care Registered Nurse (RN) offers support for our prescribers, delegation, and support for medical assistants, direction, and support to Licensed Practical Nurses (LPN) and provides integrated primary care and behavioral health nursing services for patients (across the life span) within the Health Center Program. Duties will include but are not limited to health assessment, treatment planning, patient wellness education and support, phone call triage, medication administration/management and lab testing. This RN will engage as a member of a collaborative integrated care team, acting as a Charge Nurse within the psychiatry and/or primary care departments as needed. With a Trauma Informed approach, this provider will provide nursing support and assistance to the members of the interdisciplinary team in carrying out nursing interventions and the Individualized Treatment Plan. The Integrated Care RN will work within scope of practice providing evidence-based treatment to patients and families using an integrated model of care to maximize recovery for holistic wellness. Services may be rendered onsite at any Crossroads Health location, including North Coast House, either in person, virtually or at other locations in the community as indicated needed during staffing crisis.

Requirements

  • Associates degree in nursing required. Bachelor’s degree preferred
  • Licensed in the State of Ohio with an RN
  • 1 year experience required. 3-5 years preferred in primary and psychiatric care preferred
  • Working with patients who have mental health conditions.
