About The Position

Broad Statement of Responsibilities: The Integrated Care Advisor will support clinical operations from client intake to discharge. Serve as client Integrated Care Advisor and communicate with clinical, medical, and hospitality. Provide support for client legal issues during their stay in treatment. Requirements

  • 2+ years in Behavioral Health
  • Able to stand for 8+ hours
  • Able to squat, bend, kneel, and lift 50 lbs
  • Knowledge of various computer programs and the ability to multitask

  • Basic computer skills are required with Microsoft Office Suite and web-based products knowledge a bonus.
  • Experience with EMR systems – Kipu a bonus.
  • Routine mobility to include standing, stooping, bending, lifting, walking and running.
  • Regularly required to sit and work at a phone/computer work station
  • Ability to lift up to 50 lbs.

Responsibilities

  • Oversees and Facilitates client clinical intake process
  • Responsible for entering all pertinent notes in Kipu
  • Develop the client case. Must gain a deep understanding of client’s clinical, medical, legal, and personal development needs
  • Ability to complete and organize client Biopsychosocial and other assessments
  • Work with clinicians, community relations, referent, family, and outside providers to develop client’s clinical discharge plan
  • Work closely with Client Experience Coordinator to meet clients’ needs
  • Work closely with medical, psychological testing/outcomes and hospitality to get client’s appointments scheduled
  • Work with attorney’s and criminal justice departments to meet client’s legal needs while in treatment
  • Support clinical department with client case management throughout treatment process
  • Work closely with Client Support Associates to maintain client care.
