About The Position

As an RN Intake Coordinator, youll play a key role in welcoming new participants into the PACE program. Youll use your clinical expertise, communication skills, and attention to detail to guide prospective participants and their families through the intake and enrollment process. This position is ideal for a nurse who enjoys working directly with seniors, coordinating across teams, and supporting program growth while maintaining a strong focus on dignity, safety, and quality care. Youll be part of a supportive interdisciplinary team that values collaboration, purpose, and continuous improvement. Schedule: First shift, full-time hours. Specific work hours will be established in consultation with your supervisor.

Requirements

  • Bachelors degree in Nursing.
  • Current, unrestricted Registered Nurse license in state of practice.
  • Minimum of two years of nursing or case management experience.
  • At least one year of experience working with the frail or elderly population, or willingness to complete extensive training.

Nice To Haves

  • Knowledge of Medicare and Medicaid regulations is a plus.

Responsibilities

  • Meet with prospective participants and conduct nursing assessments to determine PACE eligibility.
  • Explain the PACE program and complete all initial intake assessments and enrollment documentation.
  • Coordinate and complete home visits, including education, assessments, and required paperwork.
  • Communicate regularly with the Participant Advocate and interdisciplinary team to ensure complete and accurate participant information.
  • Coordinate services with State Health and Human Services personnel as required.
  • Participate in team meetings, trainings, and interdisciplinary discussions.
  • Collaborate with the marketing team on outreach and enrollment activities as needed.
  • Implement and follow established intake processes to support timely and efficient enrollment.
  • Collect, complete, and maintain confidential documentation related to participant assessments.
  • Maintain required reports and statistical data.
  • Support quality improvement initiatives and program goals.
  • Perform other duties as assigned.
