Intake Coordinator
About The Position
PURPOSE OF POSITION: Responsible for coordinating all intake interviews and assessments and maintaining a utilization rate of 90%. REPORTS TO: Intake Supervisor SPECIFIC RESPONSIBILITIES: Complete pre-screenings and intake assessments for clients in need of shelter or other services. Review assessments, Psychiatric evaluations and other documents received from referral sources. Participate in service and discharge planning and conferencing, when appropriate. Complete fieldwork and outreach to build network of potential referral sources. Report any suspected abuse or neglect to State Central Register. Act as liaison between program sites, community-based organizations, and agencies that serve runaway and homeless youth. Complete DYCD Connect (PTS) enrollments for new admissions. Work collaboratively with Case Planners and administrative team to ensure quality of service. Participate in other program activities including Quality Assurance team. Any other assigned tasks.
Requirements
- Bachelor degree in a related field is required
