About The Position

As a Registered Nurse (RN) - Hospital at Home, you'll deliver high-acuity, compassionate care to patients outside of a traditional hospital setting-making a real difference where it matters most. This role is perfect for a skilled RN who thrives in a fast-paced, independent environment and values flexibility, autonomy, and meaningful patient connections. Flexible shifts (AM, PM, Night options) allow you to deliver hospital-level care in a 1:1 patient setting with strong clinical support and an RN leadership team. You'll have the opportunity to work with advanced home care technology and build meaningful patient relationships and outcomes.

Requirements

• Active Registered Nurse (RN) license (Ohio)
• Minimum 1-2 years of RN experience
• Local candidate within 30 miles of Cleveland area
• Valid driver's license, reliable transportation, and auto insurance
• Willingness to travel between patient homes
• Current CPR certification
• Negative TB test (within 12 months)
• Up-to-date immunizations or exemptions (COVID, Flu, MMR, Hep B)
• Ability to pass background check and drug screening
• Strong clinical judgment and critical thinking
• Ability to work independently in a home setting
• Excellent communication and patient education skills
• Tech-savvy (EMR, mobile charting, telehealth tools)
• Compassionate, adaptable, and patient-focused

Nice To Haves

• Experience with IV therapy, wound care, or acute care strongly preferred
• Home health, ER, ICU, or med-surg experience preferred

Responsibilities

• Perform comprehensive in-home patient assessments
• Monitor vital signs, symptoms, and overall condition
• Identify and escalate changes in patient status
• Administer medications (oral, IV, injections)
• Perform wound care, IV therapy, catheter care, and other procedures
• Execute physician orders and individualized care plans
• Collaborate with physicians and interdisciplinary teams
• Coordinate labs, diagnostics, and additional services
• Ensure seamless care transitions and continuity
• Educate patients and caregivers on disease management and medications
• Provide guidance on recognizing warning signs and next steps
• Maintain accurate EMR documentation in real time
• Follow all regulatory, HIPAA, and company standards
• Act quickly in urgent situations and initiate appropriate interventions
• Coordinate hospital transfers when necessary
• Follow strict infection prevention protocols
• Maintain a safe and clean care environment
