About The Position

RN Case Manager – Hospice (Key West, FL) Amedisys Hospice Care This position is based in Marathon, FL and requires daily travel throughout the Lower Florida Keys. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

Requirements

  • Current RN license in the state(s) where you will work
  • Minimum 1 year of bedside RN experience in an acute healthcare setting (Required)
  • Current CPR certification

Responsibilities

  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.
