About The Position

Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. Requirements

  • Associate’s degree or equivalent, Graduate from an accredited nursing program
  • 3 years of relevant experience in Nursing
  • Michigan Registered Nurse (RN) license upon hire

Responsibilities

  • Manages and documents calls in a timely, efficient manner consistent with Hospice and Palliative Care standards.
  • Contributes to meeting service level commitments by handling calls in a manner that supports overall department goals.
  • Documents the patient history, diagnosis, medications and plan of care in electronic health record and completes any additional documentation as appropriate.
  • Obtains pertinent information in order to make the best decisions in referring to appropriate level of care and patient placement.
  • Communicates with physicians/nurses/other staff and provider office staff as necessary.
  • Connects customers to the appropriate person(s)/resources.
  • Collaborates with physicians, nurses, care managers, midlevel providers and outpatient and inpatient leadership teams to facilitate provision of care to patients and others with symptoms, referrals or other clinical questions.
  • Collaborates with physician for certification of eligibility.
  • Actively participates and contributes to a culture of quality, safety and collaboration that embodies and represents Corewell Health's core values of Excellence, Accountability, Compassion, Integrity, Respect, and Teamwork.
  • Actively participates in process improvement initiatives.
  • Uses safety behaviors to maintain a safe and high reliability workplace.

  • Formulates decisions and actions based on ethical principles, using the ANA Code of Ethics for Nurses to guide practice.
  • Educates potential patient/families in the Hospice at Home approach to end-of-life care.
  • Identifies the patient and families most immediate needs.
  • Actively participates in safety initiatives and risk mitigating measures where appropriate and completes all position and unit safety related competencies and requirements on a timely basis.
  • Performs other duties as assigned.
