The Hospice Central Intake and Triage Department is the main access point for all Providence Hospice customers (to include, but not limited to: current patients, current patient's family members, physicians, physician office staff, Inquiries, etc.). With new referrals, the Central Intake and Triage Nurse may arrange prescribed services with the physician, payor, patient/caregiver, and provider to ensure that services are provided in a timely and coordinated fashion. This position will also process triage calls from existing patients/caregivers, addressing clinical issues when appropriate and forwarding to individual team supervisors as necessary. Patient Population Served: Adult, Geriatric Consequently, the employee must be competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.
