In this role you will support the South Team covering mostly Summit and Medina counties - Medina, Brunswick, Wadsworth, Lodi, Stow, Akron, Cuyahoga Falls, Hudson, Fairlawn and Macedonia cities. You will be responsible for providing high quality nursing care to patients in the comfort of their own homes. As an RN, you will provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. After seeing patients, Home Health RNs work remotely to complete documentation, make calls, attend meetings, coordinate patient care and arrange visit schedules. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored to each individual and their experience level. This position is eligible for student loan repayment. This is a PRN position. Caregivers in this role will work an average of 48 hours every 6 weeks including a weekend shift (if needed). Holiday requirements include 1 holiday per year. Requirements

  • Graduate from an accredited school of Professional Nursing
  • Current state licensure as a Registered Nurse (RN)
  • Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Valid Ohio Driver's (VDL) license upon hire and per policy 8-175 thereafter
  • Proof of $100,000/$300,000 automobile insurance coverage upon hire and per policy 8-175 thereafter
  • One year of current experience as an RN
  • One year of experience in an acute Medical/Surgical or Home Care setting

Nice To Haves

  • Bachelor’s of Science in Nursing (BSN)
  • Familiarity with complex medical treatments
  • Experience working in homecare, med/surg, ICU and/or ED areas

Responsibilities

  • Provide direct nursing care.
  • Perform accurate and complete admission assessments and reassessments within a specified time frame.
  • Assess patients for safety risks, abuse and neglect.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Assess, document, prioritize and manage patients’ pain.
