About The Position

Registered Nurse – Home Health Case Manager $15,000 Sign-On Bonus Full time and $7,500 Part Time Do you have the desire to play a meaningful role in today's healthcare industry? The RN Case Manager is part of a healthcare team (patient, family, nurse, physician, therapist and others as applicable) that will work together to help our patients reach and maintain their highest level of independence with the goal to remain safe at home. Along with other duties, the nurse will perform direct care skilled visits according to the physician plan of care orders, complete comprehensive patient assessments, develop a plan of care in coordination with the healthcare team and communicate effectively to meet the patient’s needs. The nurse will ensure that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures.

Requirements

Valid driver's license and vehicle insurance
Graduate of an approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing

Nice To Haves

One (1) year home health experience preferred
