Requirements
- Current RN license in the state of practice.
- Minimum of 1–2 years of nursing experience; home health experience preferred.
- Strong clinical assessment, critical thinking, and communication skills.
- Ability to work independently and manage multiple patient visits efficiently.
- Valid driver’s license and reliable transportation.
- Basic Life Support (BLS) certification required; other certifications may be preferred.
Responsibilities
- Assess patient health status, develop individualized care plans, and evaluate outcomes.
- Provide nursing interventions including medication administration, wound care, and chronic disease management.
- Educate patients and families on disease management, medications, lifestyle modifications, and self -care techniques.
- Monitor and document patient progress, communicate changes to physicians, and coordinate care with interdisciplinary teams.
- Ensure compliance with infection control, safety protocols, and regulatory standards.
- Support patients and families emotionally and provide guidance on home care resources.
- Participate in quality improvement initiatives and professional development activities.
