About The Position
RN Home Health Case Manager Absolute Home Health provided skilled nursing, therapy services, social service and home health aides in the home under the direction of the patient's primary care physician. We are currently hiring an RN who has at least 2 years of home care experience. This RN will be responsible for completing admissions, routine visits, and will share on-call responsibilities. Requirements
- OASIS knowledge and competency
- 2 years of skilled home care experience
- Excellent written and oral communication skills
- Detail oriented
- Self motivated
- Work well in a team environment and independently
- Demonstrates flexible and efficient time management and ability to prioritize workload
- Computerized medical records
- Great customer service skills
