Requirements

  • Current RN license in Pennsylvania
  • Prior home health experience preferred
  • Strong knowledge of OASIS and CMS Guideline
  • Reliable transportation and strong time management

Nice To Haves

  • Prior home health experience preferred

Responsibilities

  • Complete OASIS assessments (Start of Care, Recert, Resumption, Discharge)
  • Provide skilled nursing services: wound care (including complex wounds), IV therapy, and medication management
  • Support chronic disease management and patient/family education
  • Work closely with PT/OT/ST and the full interdisciplinary team
  • Follow CMS, Medicare, and PA DOH standards with confidence and accuracy
