Requirements

  • Graduate of an accredited school of nursing.
  • 3 years of clinical experience in the delivery of patient care as a registered nurse in a diverse medical background.
  • Current registered nurse (RN) license in state of practice.
  • Current Basic Life Support (BLS) certification from the American Heart Association.
  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the required job processes.
  • Good organizational, decision-making, and delegating skills.
  • High level of interpersonal and public relation skills.
  • Leadership skills.
  • Ability to handle high pressure situations.
  • Ability to be highly motivated, eager to learn new skills, and find creative solutions to challenges.

Nice To Haves

  • Bachelor’s degree in nursing.

Responsibilities

  • Oversees the disease registry database.
  • Conducts pre-visit chart review of patients.
  • Works with patients and families on self-management support.
  • Coordinates care across the care continuum (case management).
  • Assists in developing tools and quality improvement activities.
  • Performs other related duties as required.
  • Remains knowledgeable on current federal, state and local laws, accreditation standards or regulatory agency requirements that apply to the assigned area of responsibility and ensures compliance with all such laws, regulations and standards.
