About The Position
The Emergency Care Coordinator provides care coordination, utilization review, and discharge planning to support timely and safe patient transitions from the ED.
Requirements
- Active NY RN license
- 5 years clinical RN experience
- Background in care coordination and discharge planning
- ASN/ADN
- BLS certification
Nice To Haves
- BSN or higher
- Case Management certification
Responsibilities
- Coordinate patient care in the ED
- Conduct utilization management reviews
- Plan and implement discharge processes
- Identify and resolve discharge barriers
- Ensure regulatory compliance and documentation
