Position Summary: We are seeking an experienced and dedicated RN Director of Patient Care to lead our hospice clinical team. This role is crucial in ensuring that our patients receive excellent end-of-life care while maintaining compliance with state, federal, and organizational standards. The ideal candidate will have strong leadership skills, a passion for hospice care, and the ability to mentor and inspire a multidisciplinary team.

Requirements

  • Active Registered Nurse (RN) license in CA.
  • Minimum of 3-5 years of experience in hospice or palliative care; prior leadership experience required.
  • In-depth knowledge of hospice regulations, including Medicare Conditions of Participation.
  • Exceptional organizational, communication, and leadership skills.
  • Ability to inspire and lead a diverse clinical team with professionalism and compassion.
  • Proficiency in electronic medical records (EMR) systems and Microsoft Office Suite.
  • Valid driver’s license and reliable transportation for occasional travel.

Responsibilities

  • Oversee all clinical operations, ensuring quality patient care and adherence to regulatory standards.
  • Lead, mentor, and develop a team of nurses, social workers, chaplains, and other hospice care professionals.
  • Ensure the development and implementation of individualized patient care plans in collaboration with the interdisciplinary team.
  • Monitor and evaluate patient outcomes to ensure continuous quality improvement.
  • Provide direct support and guidance to staff, fostering a collaborative and supportive environment.
  • Develop and maintain strong relationships with patients, families, and referral sources to promote care continuity.
  • Ensure compliance with Medicare, state, and federal regulations, as well as company policies and procedures.
  • Participate in budget planning and resource allocation to maintain efficient operations.
