We are growing and seeking an experienced Director of Patient Care Services (DPCS) to lead our hospice clinical team in Milpitas, CA. Requirements

  • Active California RN License
  • 5+ years of nursing experience
  • 3+ years of hospice experience (case management)
  • 3+ years of leadership/management experience in hospice care
  • Valid driver’s license and insured vehicle

Responsibilities

  • Direct and oversee all hospice clinical services and patient care operations.
  • Ensure compliance with state and federal hospice regulations.
  • Develop and implement patient care plans, collaborating with physicians and interdisciplinary teams.
  • Lead hiring, training, and performance evaluations for clinical staff.
  • Monitor operational indicators, budgets, and quality improvement programs.
  • Maintain accurate clinical records and ensure quality, safe delivery of hospice services.
  • Serve as a resource for staff education and orientation programs.
  • Act as a case manager in the field when needed.
